yes it does make you lopsided. The side that you carry the heavy bag on will tend to be a bit higher than the side you carry nothing on because you'll be used to forcing that side up to hold the weight.
Ideally, you want to distribute the weight across your back as evenly as possible.
When I was a kid, we carried hard leather packs that looked like rectangular boxes across two shoulders with wide straps. They were not as fancy looking as the saggy bags of today, but they're no longer made because they are more expensive to produce. So the young people today sacrifice health in exchange for cute looking, cheap bags.
